01About these terms

This website is operated by Artistic Dentistry, the cosmetic and family dental practice of Dr. Kami Hazaveh, DMD, at 501 Carsonia Avenue, Reading, PA 19606. In these terms, “we,” “us,” and “our” mean Artistic Dentistry, and “you” means anyone who uses the site.

Our Privacy Policy explains how the site handles information, and our Notice of Privacy Practices explains how we protect the health information of our patients. Both are available on this site and should be read together with these terms.

02General information, not dental or medical advice

Everything on this site, including text, images, animations, and descriptions of treatments, is general information provided for educational purposes. It is not dental or medical advice and is not a substitute for an examination, diagnosis, or treatment by a qualified dentist or physician. Every patient is different: a treatment described on the site may not be right for you, and results vary. Do not ignore professional advice or delay seeking care because of something you read here.

03No dentist–patient relationship

Using this site, including reading its content, does not create a dentist–patient relationship between you and Dr. Hazaveh or anyone at our practice. That relationship begins only when we agree to see you as a patient.

04Emergencies

This site is not for emergencies. If you think you have a medical emergency, such as trouble breathing, swelling that is spreading, or bleeding that will not stop, call 911 or go to the nearest emergency room right away. For an urgent dental problem, call our office at (610) 779-0911.

05Accuracy and availability of information

We try to keep the site accurate and up to date, but information about our services, hours, and other details may change without notice and may contain errors. Please call us to confirm anything that matters to you before relying on it. We may change, suspend, or remove any part of the site at any time.

06Ownership of site content

The site and its content, including text, images, graphics, 3D models, animations, sounds, logos, and design, are owned by Artistic Dentistry or by those who have licensed it to us, and are protected by copyright, trademark, and other laws.

You may view the site and print or save pages for your own personal, non-commercial use, as long as you keep any copyright and other notices. You may not otherwise copy, reproduce, modify, distribute, publicly display, or create works based on the site or its content without our prior written permission.

These terms apply to the website only. They do not limit any rights you have as a patient with respect to the dental care you receive from us, which is governed by applicable law and professional standards, and they do not exclude any liability that cannot be excluded or limited under applicable law.

11Indemnity

To the extent permitted by law, you agree to indemnify and hold harmless Artistic Dentistry, Dr. Hazaveh, and our staff from any claims, losses, and reasonable expenses, including attorneys’ fees, arising from your misuse of the site or your violation of these terms.

12Changes to these terms

We may update these terms from time to time. When we do, we will post the updated terms on this page and change the date at the top. Changes apply from the date they are posted, and if you keep using the site after that, you accept the updated terms.

13Governing law and general terms

These terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to its conflict-of-laws rules. Except where applicable law provides otherwise, any dispute arising out of or relating to these terms or the site will be brought in the state or federal courts located in Pennsylvania, and you consent to the jurisdiction of those courts.

If any part of these terms is found to be unenforceable, the rest will remain in effect. If we do not enforce a provision, that is not a waiver of our right to enforce it later. These terms, together with our Privacy Policy, are the entire agreement between you and us about your use of the site.
